b) detailed resume / Curriculum Vitae
c) highest educational certificate complete with transcripts
d) NRIC / FIN / Passport
e) relevant testimonials or reference letters
f) name of PI you are seeking internship / PhD mentorship with
g) period of internship requesting for (if applicable)
